✍️ How to use this format

A leave application needs exactly three facts: the dates you will be absent, the reason, and (for longer leave) how missed work will be covered. State them in the first two sentences — the rest is courtesy. Vague requests ('a few days') are the most common reason applications get sent back.

Submit it before the absence whenever possible; for sudden illness, send it the same morning and attach a medical certificate if your school or office requires one for longer leave. If your attendance percentage is close to the limit, check it before taking optional leave.
