A leave application needs exactly three facts: the dates you will be absent, the reason, and (for longer leave) how missed work will be covered. State them in the first two sentences — the rest is courtesy. Vague requests ('a few days') are the most common reason applications get sent back.
Submit it before the absence whenever possible; for sudden illness, send it the same morning and attach a medical certificate if your school or office requires one for longer leave. If your attendance percentage is close to the limit, check it before taking optional leave.
Address the principal or manager, add a subject with the leave dates, then state the reason and exact dates in the first sentence. Close politely and add your name, class/employee details. Attach a medical certificate for sick leave longer than 2–3 days if required.
Before, whenever the absence is planned (functions, travel, appointments). For sudden illness, an application submitted on the first day back — with the dates clearly stated — is normally accepted.
Approved leave usually still counts as absence for attendance percentage unless your institution marks it as 'on duty'. Use an attendance calculator to check whether the days you plan to take keep you above your institution's minimum (often 75%).
